Treatment protocol
7 steps
- 01
Assessment
Evaluate lip ratio (1:1.6 lower:upper as starting reference). Discuss desired aesthetic openly.

Block anaesthesia
Optional infraorbital + mental nerve blocks for sensitive patients.

Antisepsis
Chlorhexidine 2%, full perioral sterile field.
- 04
Border definition
27G needle, retrograde linear threads along vermillion. 0.05–0.1 ml per side.
- 05
Body volumisation
25G cannula entry at oral commissure. Submucosal threads, 0.1–0.2 ml per quadrant.
- 06
Cupid's bow & tubercles
Micro-boluses (0.02 ml) for projection. Mould between fingers.
- 07
Review at 2 weeks
Assess symmetry, top-up if indicated. Maximum 2 ml per session.
Contraindications
Do not treat if…
- Active herpes labialis — defer or pre-treat with antiviral
- Pregnancy / lactation
- Recent dental procedure (<2 weeks)
- Known HA hypersensitivity
Aftercare
Patient instructions
- Antiviral prophylaxis if herpes history (valaciclovir 500 mg ×2 daily, 3 days)
- Cold compress 10 min/hour for first 4 hours
- Avoid hot drinks and spicy food for 24 hours
- No lipstick for 12 hours
- Expect swelling for 48–72 hours
